Request a Certificate of Insurance

To request a COI please submit the following information to:
HalifaxCertificaterequests@Marsh.com, copied to
bmacinnis@hopa-advantage.ca.
- Legal name and full mailing address of requesting entity
- Specific reason for the request (e.g. new contract being entered into, requirement of a professional association etc.)
- If contractual, please provide a copy of the full contract for consideration
- Confirmation of coverages requested (e.g. property, general liability, professional liability etc.)
- Any specific requirements concerning the certificate (e.g. add as Additional Insured etc.)
