Request a Certificate of Insurance

For an explanation on what a certificate of insurance (COI) is and what it means, please click here.

​To request a COI please submit the following information to:
HalifaxCertificaterequests@Marsh.com, copied to
bmacinnis@hopa-advantage.ca.

  • Legal name and full mailing address of requesting entity
  • Specific reason for the request (e.g. new contract being entered into, requirement of a professional association etc.)
  • If contractual, please provide a copy of the full contract for consideration
  • Confirmation of coverages requested (e.g. property, general liability, professional liability etc.)
  • Any specific requirements concerning the certificate (e.g. add as Additional Insured etc.)
